Milk Thistle on the Label: Silymarin vs. Silibinin Standardization, the 80 Percent Standardization Trick That Hides an Unknown Isomer Mix, and the Bioavailability Formulations That Matter More Than the Milligram Count on the Front of the Bottle
Milk thistle sits in almost every liver-support formula on the shelf, and the front-of-label story is remarkably consistent — 250 mg, 500 mg, sometimes 1000 mg per capsule, 'standardized to 80 percent silymarin.' The Supplement Facts panel math tells a different story. Silymarin is not one molecule — it is a complex of six to seven flavonolignans (silibinin A and B, silicristin, silidianin, isosilibinin A and B, silimonin) with meaningfully different pharmacokinetics and hepatoprotective potencies, and silibinin (specifically silibinin A + B, the diastereoisomer pair) is the active-fraction subset the human trials are anchored on. An 80 percent silymarin standardization tells the buyer almost nothing about how much silibinin the capsule actually delivers. This post walks the silymarin-vs-silibinin distinction, the standardization ambiguity that lets 80 percent claims cover 15 to 45 percent silibinin content, the poor 5 to 10 percent oral bioavailability of raw silymarin extracts, the phytosome and Legalon-branded silybin-phosphatidylcholine complex that changes the pharmacokinetics 5 to 10-fold, and the 20-second Supplement Facts read that separates a serious hepatoprotective product from a marketing-grade milk-thistle capsule.

Milk thistle (Silybum marianum) is the most widely sold hepatoprotective herbal ingredient in the U.S. supplement market. Walk the liver-support shelf at any retailer and almost every product carries it — some as a standalone capsule at 250 to 1000 mg, most as an anchor ingredient in multi-herb "liver detox" or "liver cleanse" formulas alongside artichoke, dandelion root, turmeric, and a rotating cast of botanicals with much weaker human evidence bases. The front-of-label copy is remarkably standardized: a milligram count, the phrase "standardized to 80 percent silymarin," and some version of "supports healthy liver function" as the compliant structure-function claim.
That front-of-label story compresses a genuinely complicated bit of natural-product chemistry into a marketing bullet that hides most of the information the buyer would need to evaluate the product. Silymarin is not a single molecule. It is a flavonolignan complex — a mixture of six to seven closely related compounds (silibinin A, silibinin B, silicristin, silidianin, isosilibinin A, isosilibinin B, and silimonin) that co-occur in the milk-thistle seed extract in ratios that vary meaningfully by extraction method, seed source, and manufacturer. Not all of these compounds are equally active. The human clinical evidence for hepatoprotection — the ~40 years of trial data in chronic viral hepatitis, alcoholic liver disease, non-alcoholic fatty liver, and toxin-induced hepatotoxicity — is anchored on silibinin specifically, and more precisely on the silibinin A + B diastereoisomer pair.
An 80 percent silymarin standardization claim means the extract is 80 percent flavonolignan complex by weight. It does not specify how much of that 80 percent is silibinin versus the other five or six compounds in the complex. Depending on the extract source and manufacturing choices, an 80 percent silymarin extract can carry silibinin content anywhere from ~15 percent to ~45 percent of the total capsule weight. Two products with identical front-of-label copy ("500 mg milk thistle, standardized to 80 percent silymarin") can deliver a 3x range of the actual clinically-relevant compound.
That is the standardization problem. The bioavailability problem sits on top of it. Raw silymarin extract, delivered as a plain powder-fill capsule, has a poorly absorbed oral pharmacokinetic profile — roughly 5 to 10 percent oral bioavailability by most published pharmacokinetic studies. The pharmaceutical-grade formulations used in the flagship trials (Legalon in Europe, silybin-phosphatidylcholine phytosome complexes) reformulate silibinin into a phospholipid-bound complex that improves oral bioavailability 5 to 10-fold. Two products at the same silibinin milligram count can deliver an order-of-magnitude difference in plasma silibinin exposure. Nothing on the front of a typical retail bottle communicates this.
This post walks the flavonolignan-complex chemistry, the 80 percent silymarin standardization ambiguity, the bioavailability question the marketing does not surface, and the 20-second Supplement Facts read that separates a serious hepatoprotective product from a marketing-grade milk thistle capsule.
What Silymarin Actually Is
Silymarin is the collective name for the flavonolignan complex extracted from milk thistle seed. The complex contains, in typical extracted proportions:
- Silibinin (also spelled silybin): 50 to 70 percent of the flavonolignan complex. Silibinin is a 1:1 diastereoisomeric mixture of silibinin A and silibinin B. This is the primary pharmacologically active compound and the substance the clinical trial literature is anchored on.
- Isosilibinin (isosilybin) A and B: 5 to 15 percent. Structural isomers of silibinin with similar but somewhat weaker activity in the in vitro liver-cell models.
- Silicristin (silychristin): 15 to 25 percent. Active in the antioxidant and hepatocyte membrane-stabilization assays; less studied in human trials.
- Silidianin (silydianin): 5 to 15 percent. Active in some in vitro assays; largely unstudied clinically.
- Silimonin and other minor flavonolignans: 1 to 5 percent.
The composition varies by seed source, extraction solvent, and manufacturer. A pharmaceutical-grade extract (Legalon's silibinin-enriched fraction) is engineered to concentrate silibinin A + B specifically, delivering a much higher silibinin percentage of total complex than a commodity 80 percent silymarin powder. A cheap 80 percent silymarin extract sourced from a bulk botanical supplier can deliver anywhere in the 30 to 55 percent silibinin range within its silymarin fraction.
The consequence for the label read: "500 mg milk thistle standardized to 80 percent silymarin" means 400 mg of flavonolignan complex per capsule. How much of that 400 mg is silibinin depends entirely on the specific extract used — anywhere from ~120 mg to ~280 mg on the wide-side range, more typically 200 to 240 mg on a middle-of-market extract. Two products with identical front-of-label copy can be delivering meaningfully different amounts of the active compound. Only the products that specifically declare silibinin content (not just silymarin content) let the buyer verify.
The 80 Percent Silymarin Standardization Trick
The 80 percent silymarin standardization spec is the market default. Nearly every retail milk thistle product carries some version of it. The spec dates to the Legalon reference standard used in early European clinical work — the original Madaus (later Meda, later Mylan) branded formulation was standardized to 70 to 80 percent silymarin as the raw-material spec that fed into the drug-formulation process. When the ingredient migrated to the U.S. supplement market in the 1990s and 2000s, the 80 percent silymarin standardization traveled with it as the default supplier spec.
What the spec does not do is standardize the silibinin content within the silymarin fraction. Two 80 percent silymarin extracts can deliver:
- 55 percent silibinin within the flavonolignan complex (a pharmaceutical-adjacent extract)
- 30 percent silibinin within the flavonolignan complex (a commodity bulk-botanical extract)
The buyer reads the same "500 mg, 80 percent silymarin" label on both and has no visibility into which one is in the capsule. The Supplement Facts panel typically declares "Milk thistle extract (Silybum marianum) (seed) [Standardized to 80% silymarin] 500 mg" without further speciation. Only a small subset of premium brands additionally declare the silibinin percentage or milligram content on the label. Fewer still declare the silibinin A + B ratio or the isosilibinin A + B ratio.
The label read that separates a transparent product from a marketing product: does the Supplement Facts panel declare silibinin content in addition to silymarin content? A product that says "500 mg milk thistle extract standardized to 80 percent silymarin (400 mg) yielding 240 mg silibinin (silibinin A + B)" tells the buyer exactly what they are paying for. A product that says only "500 mg milk thistle extract standardized to 80 percent silymarin" is under-disclosing.
The Bioavailability Problem
Raw silymarin, delivered as a plain powder-fill capsule, has poor oral bioavailability. The lipophilic flavonolignan structure resists dissolution in the aqueous gut lumen, undergoes rapid Phase II conjugation (glucuronidation and sulfation) in the intestinal wall and liver before reaching systemic circulation, and is efficiently excreted in bile. The net result across the published human pharmacokinetic studies: an oral bioavailability of roughly 5 to 10 percent for standard silymarin extracts, with peak plasma silibinin concentrations in the low-nanomolar to low-micromolar range at typical 200 to 400 mg silibinin doses.
The pharmaceutical-grade formulations used in the flagship European clinical trials (Legalon Sil for intravenous use, Legalon oral for outpatient hepatitis and NAFLD trials, IdB 1016 as the silibinin-phosphatidylcholine phytosome complex used in a substantial fraction of the human bioavailability work) address the bioavailability problem through pharmaceutical formulation:
- Silybin-phosphatidylcholine phytosome complex (Siliphos, IdB 1016). Silibinin bound to phosphatidylcholine in a 1:1 or 1:2 molar ratio. The phospholipid pairing improves aqueous dispersion in the gut, reduces first-pass conjugation, and improves oral bioavailability 5 to 10-fold compared to plain silymarin at matched silibinin dose. This is the formulation that appears in most of the peer-reviewed silibinin PK studies from 2000 onward and in a growing fraction of the retail premium products.
- Silibinin-cyclodextrin inclusion complexes. Less common on the retail shelf; delivers a similar bioavailability improvement through a different molecular-encapsulation mechanism.
- Nanoparticle and self-emulsifying delivery systems. Emerging in the pharmaceutical literature; not yet common on retail shelves.
A retail milk thistle product that uses the phytosome delivery system typically declares it explicitly — "Siliphos" or "silybin-phosphatidylcholine complex" or "silybin phytosome" on the label — because the formulation is patented, branded, and more expensive to source. A product that does not declare it is almost certainly using plain silymarin extract with the 5 to 10 percent bioavailability floor.
The bioavailability question dominates the milligram-count question. A 240 mg silibinin dose in phytosome form delivers plasma silibinin exposure comparable to a 1,200 to 2,400 mg silibinin dose in plain extract form. Two products at very different price points can be delivering similar plasma exposure — or wildly different plasma exposure at the same price point. The label read has to go past the milligram count.
The Studied Dose Bracket
The clinical trial doses for the endpoints where milk thistle has any credible evidence base:
- Chronic viral hepatitis (hepatitis C, hepatitis B) and non-alcoholic fatty liver disease. Silibinin 140 to 420 mg per day, typically split into 2 or 3 doses. In phytosome form this delivers plasma silibinin exposure roughly equivalent to the 1,000 to 2,000 mg per day of plain silymarin used in the older trials. The efficacy signal in the meta-analyses is modest — mixed results on ALT and AST reduction, more consistent signal on markers of oxidative stress and fibrosis in some subgroups, no reliable effect on viral load or disease progression in the highest-quality trials.
- Alcoholic liver disease. Silibinin 420 to 600 mg per day in the phytosome or Legalon-branded forms. Small effect sizes on liver enzymes; no clear effect on all-cause mortality in the highest-quality Cochrane review.
- Amanita phalloides mushroom poisoning (the one indication with high-confidence evidence). Intravenous silibinin (Legalon Sil) at 20 mg/kg/day. This is a genuine emergency-medicine intervention; oral supplementation is not the route used in this indication.
- General "liver support" in otherwise healthy adults. No high-quality human trials support a specific dose or a specific benefit endpoint. This is the marketing indication, not a clinical indication.
A retail bottle sitting on the "liver detox" shelf is being purchased by a buyer who is almost never in one of the studied populations. The buyer is a healthy adult who wants to "support" or "detox" or "cleanse" the liver — a marketing frame that maps to no specific clinical endpoint. The honest position: milk thistle has a real, modest evidence base in specific hepatic disease states, and no reliable evidence base in the healthy-adult supplement-shelf indication. That is not a reason to condemn the ingredient. It is a reason to be sober about what the label is promising versus what the evidence supports.
The Multi-Herb Liver-Detox Formula Problem
A meaningful fraction of the retail milk thistle market moves through multi-herb "liver detox" or "liver cleanse" formulas rather than standalone milk thistle capsules. The typical multi-herb formula lists 8 to 15 ingredients on the Supplement Facts panel, with milk thistle at 100 to 250 mg per capsule (well below the studied dose brackets above), paired with artichoke extract, dandelion root, turmeric, N-acetylcysteine, alpha-lipoic acid, and a rotating cast of botanicals with much weaker human evidence bases in liver-specific indications.
The pixie-dusting problem — anchor ingredient at a subclinical dose to earn the label claim, paired with a long list of secondary ingredients that fill the panel and the marketing story without contributing meaningfully to the effect. A "liver detox" formula with 150 mg of milk thistle at 80 percent silymarin (120 mg silymarin, ~72 mg silibinin, in plain-extract form with 5 to 10 percent bioavailability) is delivering roughly 4 to 7 mg of plasma silibinin exposure — well below what any of the studied endpoints require. The formula is selling the milk-thistle story at a homeopathic dose.
The stack-toxicity problem — some ingredients that co-appear in liver-detox formulas (chaparral, kava, comfrey, high-dose green tea extract) carry documented hepatotoxicity signals of their own. A formula sold on liver-support framing that contains a known hepatotoxic ingredient is a genuine safety concern; the buyer trusting the front-of-label story may not be reading the full Supplement Facts panel with the required skepticism.
The label read: a standalone milk thistle product at a defensible silibinin dose in phytosome form is a more transparent choice than a multi-herb "liver detox" formula at pixie-dusted milk thistle content paired with less-studied co-ingredients.
The 20-Second Supplement Facts Read
Four questions for any milk thistle product on the shelf:
One: does the label declare silibinin content, or only silymarin content? A product that declares "yields X mg silibinin" or "standardized to Y% silibinin" is telling the buyer what they are actually paying for. A product that declares only "80% silymarin" is under-disclosing the composition of the flavonolignan complex.
Two: does the label declare the delivery form? "Silybin-phosphatidylcholine complex," "Siliphos," or "silybin phytosome" indicates the pharmaceutically-formulated bioavailability-enhanced form that dominates the human PK literature. Absence of a delivery-form declaration is a strong signal that the product is plain silymarin extract with the 5 to 10 percent bioavailability floor.
Three: does the silibinin dose match a studied range? 140 to 420 mg silibinin per day (in phytosome form) or the roughly equivalent 1,000 to 2,000 mg silymarin per day (in plain-extract form) is the studied bracket. A product delivering 100 mg of a plain silymarin extract per capsule at a one-per-day dose is delivering roughly a 5 to 10 mg plasma silibinin equivalent — well below the studied range regardless of what the front-of-label story implies.
Four: is the product standalone or part of a multi-herb liver-detox formula? A standalone milk thistle product at a defensible dose in phytosome form is a more transparent purchase than a 12-herb "liver cleanse" formula with pixie-dusted milk thistle content and additional co-ingredients that dilute the actual milk-thistle dose to a subclinical level.
Where This Leaves the Buyer
Milk thistle is not a hype ingredient invented by the last decade of supplement marketing. It has a real, decades-long clinical evidence base in specific hepatic disease states, and pharmaceutical-grade formulations of silibinin are used as a genuine emergency-medicine intervention in amanita poisoning. The problem is not the ingredient. The problem is that most retail products are engineered around the front-of-label "80 percent silymarin" story rather than around the silibinin content and bioavailability that determine what actually reaches the liver.
The transparent buyer's move: skip the "liver detox" and "liver cleanse" formulas entirely (the multi-herb, pixie-dusted, marketing-anchor category), read the Supplement Facts panel on any standalone milk thistle product for silibinin content and delivery form, prefer a phytosome-formulated silibinin product at a studied-range dose over a plain silymarin extract at a matched milligram count, and treat the healthy-adult "liver support" indication as a marketing frame rather than a clinical one. A milk thistle supplement is not a substitute for the actual liver-protective interventions (limiting alcohol, treating chronic hepatitis, managing metabolic risk factors for NAFLD) — those are the levers with the real-world effect size that no capsule matches.
The label tells the buyer what the manufacturer wants them to see. The Supplement Facts panel tells them what is actually in the bottle. Read the panel.
